We offer a range of dental services which can be performed onsite at long-term care facilities, including preventive care, restorative dentistry, oral surgery and dentures. The specific services provided to you will be determined by your loved one’s individual needs and discussed with the patient and/or with you by our dental professionals, including our office staff.

2. Appointments

• Referrals are provided by the long-term care facilities. No need to make appointments directly with us. Your loved ones will be seen when the long-term care facility is on our schedule.

3. Financial Policy

• Please take a look at the financial responsibility form which you must sign regarding our financial policy. You can request from us by emailing us at office@godentalcare.org.

4. Patient Information

• Accuracy: You are responsible for providing accurate and complete information, including your medical history, insurance information, and contact details.
• Privacy: We are committed to protecting your privacy. Your information will be handled in accordance with HIPAA regulations and our Privacy Policy.
• Updates: Please notify us of any changes to your personal information, including your address, phone number, or insurance coverage.
